We are seeking a qualified and experienced Therapeutic Case Manager to join our friendly and supportive Enhanced Care Opportunities (ECO) team at our Dandenong office.
We are looking for someone with strong leadership and supervision experience who is able to effectively engage with and work with disadvantaged young people, their caregivers and family members.
The Therapeutic Case Manager will oversee the care team of specialist professionals in delivering high level of care for complex foster care young people in ECO.
This part time role has a focus on coordinating the care team involved with each of our young people to ensure enhanced levels of care.
The Therapeutic Case Manager will support young people and their carers and family members. The position objectives are:
